Responsibilities
- Coordinate patient care plans and treatment schedules with multidisciplinary teams
- Conduct thorough patient assessments and maintain accurate medical documentation
- Facilitate seamless communication between patients, families, and health care providers
- Implement and monitor infection control protocols and safety procedures
- Provide health education and resources to promote preventive care
- Collaborate with community partners to expand patient access to services
Qualifications
- Associate's degree in Health Science or related field (Bachelor's preferred)
- Current California Health Care Coordinator certification
- Minimum 2 years clinical or administrative health care experience
- Proficiency with electronic health records (EHR) systems
- Exceptional communication and interpersonal skills
- Ability to thrive in fast-paced, patient-centered environments
- Valid CPR certification and clean background check
