Responsibilities
- Coordinate patient care plans between departments and external providers
- Conduct initial patient assessments and document medical histories
- Assist with discharge planning and post-visit follow-ups
- Collaborate with multidisciplinary teams to ensure seamless care delivery
- Maintain accurate electronic health records and documentation
- Provide patient education on treatment protocols and preventive care
Qualifications
- Associate's degree in healthcare or related field required
- 2+ years of healthcare coordination experience preferred
- Certified Medical Assistant (CMA) or similar certification a plus
- Strong communication and interpersonal skills
- Proficiency in EHR systems (Epic experience preferred)
- Ability to work flexible hours including occasional weekends
- Basic knowledge of HIPAA compliance protocols
