Responsibilities
- Provide direct patient care including vital signs monitoring and medication assistance
- Assist with daily living activities and mobility support for elderly patients
- Maintain accurate patient records and documentation in compliance with HIPAA
- Collaborate with nursing staff to develop and implement care plans
- Support patient education on health management and preventive care
- Ensure a clean, safe, and welcoming environment for patients and visitors
Qualifications
- High school diploma or equivalent (CNA certification preferred)
- Minimum 1 year experience in healthcare or assisted living setting
- Current CPR/BLS certification
- Ability to lift up to 50 pounds and stand for extended periods
- Strong communication skills with empathy and patience
- Reliable transportation to multiple facility locations
- Pass background check and drug screening
