Responsibilities
- Coordinate patient appointments and healthcare services across local clinics and hospitals
- Act as primary liaison between patients, providers, and insurance companies
- Maintain accurate electronic health records and documentation
- Conduct community outreach to promote preventive healthcare initiatives
- Collaborate with social services to address patient barriers to care
- Monitor patient progress and adjust care plans as needed
- Train patients on chronic disease management and medication adherence
Qualifications
- Associate's degree in healthcare administration or related field (Bachelor's preferred)
- Minimum 2 years of experience in healthcare coordination or patient navigation
- Proficiency with EHR systems (Epic or Cerner experience a plus)
- Strong bilingual skills in English and Spanish
- Certified Medical Administrative Assistant (CMA) or equivalent certification
- Knowledge of local healthcare resources and community programs
- Exceptional communication and conflict resolution abilities
