Responsibilities
- Coordinate patient care plans and treatment protocols
- Collaborate with multidisciplinary healthcare teams
- Manage patient documentation and electronic health records
- Provide health education and support to patients/families

Qualifications
- Associate's degree in healthcare or related field required
- 1+ years of healthcare coordination experience preferred
- Certification in CPR and basic life support

- Valid Pennsylvania healthcare provider license
