Responsibilities
- Provide direct patient care including vital sign monitoring and medication administration
- Assist with patient mobility, hygiene, and daily living activities
- Maintain accurate electronic health records and documentation
- Collaborate with interdisciplinary care teams to coordinate patient services
- Educate patients on health management and preventive care
- Ensure clinic sanitation and safety protocols are followed
- Support telehealth initiatives and virtual patient consultations
Qualifications
- High school diploma or equivalent (college degree preferred)
- Current CPR/BLS certification (or willingness to obtain immediately)
- Ability to pass background check and drug screening
- Strong communication and interpersonal skills
- Basic computer proficiency with EMR systems
- Flexibility to work rotating weekends and holidays
- Valid California driver's license (for travel between sites)
- Spanish bilingual skills highly desirable
