Responsibilities
- Coordinate patient care plans and service delivery across departments
- Collaborate with medical teams to ensure seamless patient transitions
- Manage electronic health records and documentation systems
- Conduct patient intake assessments and eligibility screenings
- Facilitate communication between patients, families, and healthcare providers
- Monitor compliance with healthcare regulations and protocols
Qualifications
- Associate's degree in healthcare or related field (Bachelor's preferred)
- Minimum 2 years healthcare coordination experience
- Strong knowledge of HIPAA and healthcare compliance standards
- Proficient with EHR systems (Epic experience a plus)
- Excellent interpersonal and communication skills
- Ability to work flexible shifts including evenings/weekends
