Responsibilities
- Coordinate patient care plans and appointments across multiple departments
- Act as primary liaison between patients, families, and healthcare providers
- Maintain accurate electronic health records and documentation
- Facilitate smooth transitions between care settings (hospital to home)
- Provide health education resources to patients and families
- Collaborate with interdisciplinary care teams to optimize outcomes
- Monitor patient progress and adjust care plans as needed
- Address patient concerns and resolve care coordination issues
Qualifications
- Associate's degree in Healthcare Administration or related field required
- Minimum 2 years experience in healthcare coordination or similar role
- Strong knowledge of healthcare regulations and HIPAA compliance
- Excellent communication and interpersonal skills
- Proficient in electronic health record systems (EHR)
- Ability to multitask in fast-paced environment
- Certified Medical Administrative Assistant (CMA) preferred
- Valid Minnesota driver's license required for home visits
