Responsibilities
- Coordinate patient care plans across Dallas-area medical facilities
- Urgently respond to high-priority patient referrals within 24 hours
- Build relationships with Dallas hospitals, clinics, and community partners
- Manage electronic health records and documentation compliance
- Lead patient intake and eligibility verification processes
- Develop service pathways for underserved Dallas communities
- Track outcomes and optimize care coordination workflows
Qualifications
- Bachelor's degree in Healthcare Administration or related field
- 3+ years of healthcare coordination experience
- Current Texas RN license or healthcare certification required
- Proficiency with Epic EMR systems
- Exceptional communication skills in English/Spanish
- Ability to work flexible hours including weekends
- Experience with Dallas County health programs preferred
- Certified Medical Home Coordinator (CMHC) a plus
