Responsibilities
- Coordinate patient care plans and treatment schedules across multidisciplinary teams
- Facilitate seamless communication between patients, families, and healthcare providers
- Manage electronic health records with precision and maintain strict HIPAA compliance
- Conduct patient intake assessments and develop personalized care pathways
- Collaborate with community resources to support holistic patient wellness
- Monitor treatment outcomes and implement evidence-based interventions
- Train new staff on clinical protocols and patient care standards
Qualifications
- Associate's degree in Healthcare Administration or related field (Bachelor's preferred)
- Minimum 2 years clinical coordination experience in ambulatory care
- Current CPR/BLS certification from recognized provider
- Proficiency in Epic EHR systems and Microsoft Office Suite
- Exceptional interpersonal skills with demonstrated empathy
- Ability to manage competing priorities in fast-paced environments
- Virginia state health coordinator license (or ability to obtain within 30 days)
- Spanish bilingual proficiency a significant plus
