Responsibilities
- Provide direct patient care including vital sign monitoring, medication assistance, and personal hygiene support
- Assist with daily living activities for elderly and disabled patients in home and facility settings
- Maintain accurate medical records and documentation following HIPAA guidelines
- Collaborate with healthcare teams to develop and implement personalized care plans
- Transport patients to medical appointments and community activities
- Educate patients and families on health management and preventive care
- Participate in emergency response protocols and crisis intervention
Qualifications
- Current California CNA or HHA certification required
- Minimum 6 months of hands-on patient care experience
- BLS/CPR certification (or willingness to obtain within 30 days)
- Valid California driver's license with clean driving record
- Ability to lift 50+ pounds and stand for extended periods
- Strong communication skills in English (Spanish bilingual preferred)
- Pass background check and drug screening
- Flexibility to work evenings, weekends, and holidays
