Responsibilities
- Coordinate patient care plans across multidisciplinary teams
- Manage electronic health records with precision and confidentiality
- Facilitate communication between patients, families, and medical staff
- Ensure compliance with HIPAA and healthcare regulations
- Train and onboard new healthcare personnel
- Monitor patient outcomes and adjust care protocols accordingly
- Collaborate with insurance providers for seamless billing processes
Qualifications
- Associate's degree in healthcare or related field (Bachelor's preferred)
- 2+ years of healthcare coordination experience
- Certification in Healthcare Management (CHM) or equivalent
- Proficiency with EHR systems (Epic, Cerner, etc.)
- Strong interpersonal and conflict resolution skills
- Valid Pennsylvania nursing license (or willingness to obtain)
- Ability to work flexible shifts including weekends
