Responsibilities
- Provide direct patient care and support services to elderly and disabled clients
- Assist with daily living activities including medication management
- Monitor patient health status and document observations
- Coordinate care plans with interdisciplinary teams
- Maintain HIPAA-compliant medical records
- Transport patients to medical appointments
- Respond to emergency situations following protocols
- Participate in ongoing training and certification programs
Qualifications
- High school diploma or equivalent (CNA certification preferred)
- Minimum 1 year healthcare experience required
- Valid Ohio driver's license with clean record
- Ability to lift 50 pounds and stand for extended periods
- Pass background check and drug screening
