Responsibilities
- Coordinate patient care plans across multidisciplinary teams
- Manage electronic health records with precision and confidentiality
- Conduct patient intake assessments and eligibility screenings
- Liaise between healthcare providers, insurance agencies, and patients
- Track and report key performance metrics for service quality
- Facilitate community health outreach programs
- Maintain compliance with HIPAA and healthcare regulations
Qualifications
- Associate's degree in healthcare administration or related field (Bachelor's preferred)
- 1+ years of healthcare coordination experience
- Proficiency with EHR systems (Epic experience a plus)
- Certified Medical Administrative Assistant (CMA) or equivalent
- Exceptional communication and conflict resolution skills
- Ability to prioritize tasks in high-pressure situations
- Valid Illinois driver's license and reliable transportation
