Responsibilities
- Coordinate patient care plans and schedules across multiple departments
- Facilitate communication between patients, families, and healthcare providers
- Manage electronic health records with precision and confidentiality
- Assist with patient admissions, discharges, and transitions of care
- Train new staff on healthcare protocols and systems
- Monitor inventory and order medical supplies as needed
- Participate in quality improvement initiatives
Qualifications
- Associate's degree in healthcare or related field (Bachelor's preferred)
- 2+ years of healthcare coordination or clinical experience
- Current CPR certification and state healthcare license
- Proficiency in EHR systems (Epic or Cerner a plus)
- Exceptional communication and interpersonal skills
- Ability to work flexible shifts including weekends
- Strong attention to detail and organizational abilities
- Local Portland area residency required
