Responsibilities
- Provide direct patient care including vital sign monitoring, medication administration, and wound care
- Assist patients with daily living activities (ADLs) including mobility support and personal hygiene
- Document patient observations and care activities accurately in electronic health records
- Coordinate with nursing staff and physicians to ensure seamless patient care delivery
- Maintain clean and organized patient environments following infection control protocols
- Respond to patient inquiries and provide emotional support to patients and families
- Participate in weekend team huddles and safety compliance reviews
Qualifications
- Current California CNA certification required
- Minimum 1 year of clinical experience in a hospital or long-term care setting
- BLS/CPR certification from American Heart Association
- Ability to work independently and as part of a collaborative team
- Strong attention to detail and excellent communication skills
- Flexibility to work Saturdays and Sundays with rotating holiday coverage
- Proficiency with electronic health record systems (EHR)
