Responsibilities
- Coordinate patient care plans with local healthcare providers and specialists

- Conduct eligibility screenings for Medicaid and Medicare programs
- Maintain accurate electronic health records using Epic software

Qualifications
- Associate's degree in Healthcare Administration or related field
- Minimum 2 years of experience in healthcare coordination
- Certified Medical Administrative Assistant (CMAA) preferred

- Strong interpersonal skills with diverse community populations
- Knowledge of Pennsylvania healthcare regulations
