Responsibilities
- Provide direct patient care assistance including vital sign monitoring and mobility support
- Document patient observations and care activities in electronic health records
- Assist with patient education and discharge planning coordination
- Maintain sterile environments and infection control protocols
- Collaborate with interdisciplinary care teams to ensure continuity of care
- Respond to patient call lights and address basic patient needs
- Participate in quality improvement initiatives and training programs
Qualifications
- High school diploma or equivalent required; CNA certification preferred
- Minimum 1 year healthcare or direct patient care experience
- Current CPR/BLS certification from recognized provider
- Ability to lift 50 pounds and stand for extended periods
- Strong communication skills and attention to detail
- Proficiency with EMR systems (Epic experience a plus)
- Pass background check and drug screening
- Fluency in English (Spanish bilingual a plus)
