Responsibilities
- Coordinate patient care plans with multidisciplinary teams
- Manage electronic health records and documentation
- Assist with patient intake and triage procedures
- Provide health education to patients and families
- Collaborate with insurance providers for billing
- Maintain HIPAA compliance in all communications
- Support community outreach initiatives
Qualifications
- Associate's degree in healthcare or related field
- 1+ years clinical coordination experience
- Strong communication and organizational skills
- Certified Medical Assistant (CMA) preferred
- Proficient with EHR systems (Epic a plus)
- Ability to work flexible shifts
- Spanish bilingual a strong advantage
