Responsibilities
- Coordinate patient care plans between healthcare providers and facilities
- Conduct eligibility screenings for medical assistance programs
- Maintain detailed electronic health records in compliance with HIPAA
- Facilitate patient education on preventive care resources
- Collaborate with community health organizations for outreach programs
- Track and analyze healthcare access metrics for service improvement
- Provide bilingual support (English/Spanish) to diverse patient populations
Qualifications
- Associate's degree in healthcare administration or related field
- Minimum 2 years of healthcare coordination experience
- Proficiency in electronic health record systems (Epic preferred)
- Bilingual proficiency in English and Spanish
- Knowledge of Texas healthcare regulations and Medicaid programs
- Strong organizational skills with attention to detail
- Certified Medical Administrative Assistant (CMAA) certification preferred
